Hundreds of Wirral patients benefit from care at home this winter

Members of the Wirral community are benefiting from more integrated health and social care, through Integrated Care Coordination Hubs (ICCHs), a joint project led by Wirral Community NHS Trust in partnership with Wirral Council.

“We’re organising care to meet the needs of patients and their carers and improving the flow of information between community and social care providers.”

Based in Birkenhead, Wallasey, South Wirral and West Wirral; their work is a great example of the new Healthy Wirral initiative, and a key part of delivering the Wirral Plan to create a joined up system of care which supports everyone in our community to keep themselves well and live their lives in as full a way as possible.

The integrated community care teams (ICCTs) within each hub are already working with patients to help provide quicker, effective and more joined up care, speeding up access to community nurses, physiotherapists, occupational therapists, adult social services and mental health services, with the emphasis on avoiding unnecessary hospital admissions.

Speaking about the work of the ICCTs, Wirral Community NHS Trust’s Director of Integration and Partnerships, Val McGee said: “The trust’s ambitions to realise our long term vision of true integration of services is becoming a reality. We’re organising care to meet the needs of patients and their carers and improving the flow of information between community and social care providers.”

Wirral Council’s Director of Adult Social Services, Graham Hodkinson said: “Through our work with Wirral Community NHS Trust we are building on developments in services that are already having positive impacts on patients’ lives. People are receiving the right care in the right place at the right time and are at the centre of any care planning they need.”
